The Russian economy as a whole and its regional economic systems should be resilient to external challenges in the conditions of turbulence. It is important both to assess the economic sustainability of an individual region and to conduct interregional comparisons of economic sustainability within macroregions since regional economies react differently to crisis phenomena. The article assesses the economic sustainability of the Northwestern Federal District regions to external shocks in 2020–2022 in two stages. The first stage shows the reaction of the economic systems of the Northwestern Federal District subjects to external challenges based on the dynamics of GRP growth rates; the second stage presents changes in the sectoral structure. The paper reveals that structural shifts are characteristic of both stable and unstable regions. The level of structural differences varies both between regions and in relation to shocks. Regions that are resistant to both shocks (2020–2022) were not identified, while five subjects of the district proved to be unstable to the challenges of both crises. The practical significance of the work lies in the possibility of using the obtained data to comprehend regional policy, make anti-crisis decisions and develop measures to improve sustainable economic growth of the Northwestern Federal District subjects
